Suspend Payroll Deductions

This form is for employees who want to place their parking deductions on hold. Suspensions are granted for no less than one month and no more than one year. Medical Residents must return their hangtag and suspend their parking assignment in person to one of the offices located on the West Campus.

I agree to the policy below.
While my assignment is suspended, I understand I will not lose my parking assignment nor wait list priority. The suspension is for one year only. After one year, unless notified by the Parking Office, my parking assignment will be automatically canceled, and I will be removed from all wait lists and will not have priority for any on campus parking assignments. I am responsible for returning the hangtag. If the hangtag is not returned in 30 days, I will be accessed and agree to pay the $20 fee.
